Health Department Immunization Clinic Moving

Effective Wednesday, January 4, 2012, the Lake County Health Department's Immunization Clinic located at 18 North Eustis Street in Eustis will close. Beginning Monday, January 9th, immunizations will be offered at the following clinic locations:

Women's Wellness Center, 2113 West Griffin Road, Leesburg, 352-360-6548

Umatilla Clinic, 249 East Collins Street, Umatilla, 352-771-5500

Clinic hours of operation are Monday - Friday, 8:00 AM - 5:00 PM and appointments can be made by calling either the Umatilla or Leesburg offices.
